This place I bought it from has all natural stuff and it : seemed fresh in that it was sold in bulk, loose, and not packaged. No, you WANT packaged. You want that stuff away from light, because light deteriorates the staining quality. Air-tight foil packaging with an expiration date is best. I'm telling you, it was the quality of the henna and the length of time you left it on. Your hair colour might be a part of it, but I would bet cash that it's mostly the quality of the stuff and the length of time you let it rest.
