I, too, have been experiencing soome hair loss, and am only using body-art henna. The loss goes on for some weeks post-application, and I suspect it has nothing to do with either the henna, nor the application technique, but with biology. Some of us do shed in the autumn [my cats do], and it is my theory that the nascent growth we then have through the winter may act something like eiderdown, being closer to the scalp and therefore denser and warmer. But clumps of hair falling may point to something else, and perhaps a general health check is in order: deficiency of certain nutrients, or endocrine imbalance, can happen quite suddenly, and one symptom is alopecia [hair loss]. I don;t think it is your henna, but I do hope you sort it all out - best of luck!
